Policy Changes Effective January 1, 2023

• New policies take effect January 1, 2023. All new and existing patients must sign a new informed consent and HIPPA Document and Medicare patients must sign new ABN. 

• Self pay pricing is now $40.00 adjustment, $30.00 exam fee.  New patient’s first visit total is $70.00 when self paying.

• Insurance new patient is now $45.00 adjustment, $30.00 exam fee. New patient’s first visit total is $55.00 when using insurance.

 

• Medicare maintenance fee is now $26.61.

 

• Co-pay / payment is due at time of service.

 

• We now provide our patients with the option to participate in our patient communication system. These features include appointment confirmation via text messaging and email. Please provide us with your current phone number and email if you wish to participate. 

 

• No Show Fee of $25.00 will be applied to patients account and billed directly to them. Not his/her insurance company. Must be paid upon next appointment time. Fees cannot be charged to HSA/FSA’s.

 

• Please allow 24 hour notice for cancelling or rescheduling appointments. We work hard to see patients on time. If you are unable to keep your appointment please call us as far in advance as possible so we can reserve the time allowed for you to someone else. To avoid a $25.00 fee, please call in a timely manner.

** We understand in the event of an emergency, work related issue or sudden illness, this could infringe on the 24 hour allowance for notice. If one of these issues arises, please call within at least an hour notice with a valid excuse. **
